When requesting a prescription or prescription refill, please note the following information;

  * Please fill out all the requested information on the form

  *  The prescription will need to be approved by the doctor before we can process it

  *  If we haven’t seen your pet within the last year, we will not be able to fill your prescription request

  *  Please allow 48 hours for processing

  * We will call or e-mail you (your contact preference) when the prescription is ready. If you would like it mailed to your address you should be prepared to pay over the phone with a credit card. The cost of mailing will be added to the prescription invoice.

When requesting an appointment, please fill out all the information. We will try to fulfill your desired day and time but depending on the type of appointment your pet needs we make need to  make adjustments to your request.

This is a pdf file for clients to download and print off. We use this form to update existing client information and welcome new clients to our clinic. Printing off this form and filling it out before coming to the clinic will speed up the check in process.
